The ones I use are Easton which are strong, fairly light and reasonable price. There are different extension bars, straight, single, and double bends. I use the double bend ones.
I can't remember where I got them but here are some links:

Hi Tim - I have a set of BBB aerobars which look a lot like these:

They work well for me, although I've not tried any others to compare them with. I liked the fact they have plenty of degrees of adustment, although they are a bit of a pain to fix on and remove.
For which reason I doubt I'll be using them at all until May at the earliest so if you want to borrow them, give me a shout!

Oval Concepts do a good range of Tri-Bars with some of them been fixed to the handlebar stem front plate rather than the handlebars so they can be easily remove and so the fixing clamps don't stress the handlebars and weaken them if you do over torque the bolts.
They also come in lovely carbon fibre!
